Infant cardiac MRI using oscillatory ventilation: safe and effective

چکیده
Background Cardiac magnetic resonance imaging (CMRI) for infants and young children typically requires sedation. General anaesthesia can eliminate motion artefact and permits controlled ventilation with breath-holding during imaging to limit respiratory artefact, but this technique typically requires frequent changes in respiratory support that may impair pulmonary function, and pauses in image acquisition during recovery from breath-holding. HFOV provides ventilation with near-constant mean airway pressure, limited tidal volume, and minimal movement of chest wall and diaphragm, thus obviating the need for breath-holding during imaging. We have recently described a hybrid HFOV-anaesthesia delivery system for use during CMRI [Hoffman, 2010], but image quality has not been addressed heretofore.

Management of an infant with diffuse bullous pulmonary lesions using high-frequency oscillatory ventilation.
PURPOSE To describe the anesthetic and ventilatory management of an infant with diffuse pulmonary bullous lesions. CLINICAL FEATURES Four successive operations were scheduled for an infant with diffuse pulmonary bullous lesions. At the age of seven weeks, conventional positive pressure ventilation during laparotomy for intestinal occlusion led to arterial desaturation. This was corrected by r...
متن کاملThe role of high-frequency oscillatory ventilation in paediatric intensive care
Mechanical ventilation during acute respiratory failure in children is associated with development of ventilator-induced lung injury. Experimental models of mechanical ventilation that limit phasic changes in lung volumes and prevent alveolar overdistension appear to be less damaging to the lung. High-frequency oscillatory ventilation, using very small tidal volumes and relatively high end-expi...

OBJECTIVE To evaluate the safety and effectiveness of high-frequency oscillatory ventilation using a protocol designed to recruit and maintain optimal lung volume in patients with severe adult respiratory distress syndrome (ARDS). SETTING Surgical and medical intensive care units in a tertiary care, military teaching hospital. DESIGN A prospective, clinical study. PATIENTS Seventeen patie...
متن کاملDifferential lung ventilation via tracheostomy using two endotracheal tubes in an infant: a case report
BACKGROUND This case report presents differential lung ventilation in an infant. The aim is to define an alternative technique for performing differential lung ventilation in children. To the best of our knowledge, this is the first report of this kind. CASE PRESENTATION A 4.2-kg, 2.5-month-old Asian boy was referred to our facility with refractory hypoxemia and hypercarbia due to asymmetric ...
متن کاملHigh-frequency oscillatory ventilation after cardiac surgery: a treatment for all ages
a recent issue of Critical Care, Bojan and colleagues [1] reported their experience with early initiation of high-frequency oscillatory ventilation (HFOV) in pediatric cardiac surgery that demonstrated shorter mechanical ventilation times and intensive care stays. Acute lung injury is common in adult patients undergoing surgery, yet there is scant reporting of the intraoperative utility of HFOV...
